Certified Occupational Therapy Assistant (COTA) – Full-Time
Location: Premier at Perry Village – New Bloomfield, PA

Revital Rehab Services is hiring a Full-Time Certified Occupational Therapy Assistant (COTA) to join our partner facility, Premier at Perry Village, in New Bloomfield, PA. In this hands-on role, you’ll help residents improve their independence, function, and daily living skills—all while working in a supportive and team-focused environment.

Key Responsibilities:

Deliver occupational therapy services under the supervision of a licensed Occupational Therapist

Carry out individualized treatment plans to help residents meet functional goals

Monitor and document resident progress, reporting updates to the supervising OT

Participate in care team discussions to ensure high-quality, coordinated therapy services

Qualifications:

Associate’s degree in Occupational Therapy from an accredited program

Active COTA license in the state of Pennsylvania

Excellent communication skills and a collaborative mindset

Commitment to patient-centered, ethical care
